The fireplace has moderate levels of creosote buildup and dust and debris in the firebox and flue. Creosote built up is a safety hazard. A chimney sweep certified by the Chimney Safety Institute of America should clean and inspect the fireplace and flue before further use. The manufactured insert has damaged areas and cracks. These are a fire safety hazard. Embers can go through the cracks to the wood framing. A licensed general contractor should repair or replace.
